Pleats came back because proportions changed. Once trousers loosened through the thigh, the pleat stopped being a stylistic choice and went back to being structural, giving the extra cloth somewhere to sit cleanly rather than bunching at the hip.

Forward pleats open toward the fly and are the more traditional choice. Reverse pleats open toward the pocket and give a slightly cleaner front. Either works. A single pleat is the safer starting point, a double pleat commits harder to the volume.

Why did pleats come back?

Because trousers loosened. Once there is real width through the thigh, the pleat becomes structural again, giving the extra cloth a clean place to fold rather than bunching at the hip.

What is the difference between forward and reverse pleats?

Forward pleats open toward the fly and are the traditional choice. Reverse pleats open toward the pocket and give a slightly flatter front. Both work, and the difference is largely aesthetic.

Do pleated trousers suit everyone?

They suit most people better than expected, provided the rise is high enough. Pleats on a low rise trouser tend not to work. The pleat needs the waistband sitting near the natural waist to fall correctly.

How much break should pleated trousers have?

Little to none. A single soft break or a clean stop on the top of the shoe keeps the line. Too much length collapses the drape the pleats create.
